19 deaths at VA traced to poor care

ST. LOUIS - Substandard care at a southern Illinois Veterans Affairs hospital may have contributed to 19 deaths over the past two years, a VA official said Monday as he apologized to affected families and pledged reform.

The hospital in Marion, Ill., initially drew scrutiny over deaths connected to a single surgeon, but two federal reports found fault with five other doctors.

The hospital undertook many surgeries that its staffing or lack of proper surgical expertise made it ill-equipped to handle, and hospital administrators were too slow to respond once problems surfaced, said Dr. Michael Kussman, U.S. veterans affairs undersecretary for health.

Visy, Pratt fined $36m over price-fixing

Billionaire Richard Pratt and his Visy group companies have been fined a record $36 million for price fixing in the cardboard market, in a case the Federal Court says is the worst in Australia's corporate history.

Mr Pratt's right-hand man, former Visy chief executive Harry Debney, has been fined $1.5 million and former general manager Rod Carroll was fined $500,000. Federal Court judge Justice Peter Heerey said the penalty imposed on Visy was twice the highest previous fine imposed by his court. ``That is reflective of the fact that this must be, by far, the most serious cartel case to come before the court in the 30-plus years in which price fixing has been prohibited by statute,'' Justice Heerey said in his decision. A Visy spokesman said the company respected the decision.

Governor's Office Re-Opens In Western Mass.

In the vestibule, empty shelves soon will be filled with new brochures on state programs, grant applications, tourism and other topics of possible interest to visitors.

Cardona, 39, a Springfield native appointed in January to run the office, said she receives a steady stream of calls from throughout the four-county region.

Many people have questions about state services or suggestions on improving programs. Others need help with everything from starting a new business to understanding the state's health care insurance requirements.

As word has spread that the office reopened, she said, drop-in visits have increased, too.

"Our goal is to make this office as valuable a resource as possible," Cardona said. "The governor has clearly said his intention is to be the governor for the entire state, and I do believe he'll be using this office frequently."

Full-time branch offices are common in many larger states nationwide, Massachusetts and Connecticut are the only New England states that have them.

When the author, Margaret Forster, decided on a university, she chose Oxford where she headed with an open scholarship. Her husband, the writer Hunter Davies, who was also from Carlisle, picked Durham University. Another local luminary, the writer and broadcaster Melvyn Bragg, also selected Oxford.

None of them could have stayed at home, if they had wanted to, because until recently Cumbria had no university of its own. There was no higher education in the north westernmost corner of England beyond Lancaster, not even a college of higher education, though there was a teacher training college in Ambleside and an art college in Carlisle.

This lack in what is a very large and beautiful county stretching from Lancashire in the south to Scotland in the north, and from the Irish Sea in the west to the Pennines in the east had a profound effect on the region.
